Inspired by a hint from daggerfall's manual, the Ironman game systems mod, a judging criteria on a page full of daggerfall classic save files, and a core gameplay mechanic from a game called Vampyr, this mod would do the following:
+Prevent manual saving
+Autosave after character Creation (slot 1)
+Autosave after rest, fast travel, entering a town or dungeon, exiting a town or dungeon, and completing or failing a quest. (Slot 2)
+Delete save slot 2 on character death.
+Allow toggling of each feature or Autosave condition.
I personally like to think of this mod under the names "Live With it" or "Suffer the Consequences".

If it deletes save 2 on death, why not autosave periodically while in a dungeon? Seems like all that's accomplished by not doing so is punishing people for having flaky machines or power outages.
If the goal is to force people to live with consequences it should also save on, eg, accepting a quest, killing an NPC, getting detected breaking the law, killing an enemy, or really anything with permanentish consequences, good or bad. Otherwise nothing but death is permanent if you just exit the game.
