Having prostitution in for flavour only, with no gameplay impact beyond being a money sink, is more appropriate - to my sensibilities anyway. Unfortunately, makes it much less useful as a game system though.
We have magic healing temples, so whats wrong with magic healing whores. An option to have them non-functional in the mod menu would likely be prudent for those who don't like the system, but want the flavor.
After thinking about it, I think the best way to implement this would be for the service to only restore magicka while lowering stamina, while having a high gold cost and requiring a room to be rented at the inn. Sure you can use it to help grind out magic levels, but eventually you need to rest. If you don't have enough stamina then you could be told your too tired and to go get some rest, which would avoid passing out afterwards. This could be balanced since there are only a couple good ways to restore magicka compared to health and stamina. The one problem with this system would be casting restore stamina between sessions.
Stat restoration methods:
Stamina:Spells, potions, resting
Health:Spells, potions, resting, temples(if a member), Regenerate Health advantage/magic item
Magicka:Potions, resting, mages guild(sorcerers only), Spell Absorption advantage/magic item

With STDs, the simple solution would be a low chance of contracting a common disease (I got the Swamp Rot) the same way you get it from combat. The low chance would be a combination of the chance of the prostitute having a disease and also the chance of spreading it through contact. If anything it adds another disadvantage for using the service to help balance it out. Vampirism and Lycanthropy could be included, or maybe not. Probably not.

That would likely be built into the quest as the client is going through the guild to find something "special."

I thought about that too, but I couldn't find anything lore-friendly outside of that one DiBella priestess in Skyrim. Though there is some crossover between the two, being a DiBella follower doesn't automatically mean your a prostitute and vise versa. How the two factions interact with both quests and reputation could be considered.
