Basically, if the player sets a recall anchor in a dungeon, leaves & recalls back in there won't be any enemies present. It seems likely that a teleport quest action into a dungeon would result in the same bug, but I haven't tested that.
Thank you. A small oversight on my part, I forgot to add spawns when teleporting into dungeon as opposed to entering normally or restoring state from a save game.
