I experienced this (pretty minor) bug twice today in build 0.7.146 (Windows 10 x64). This issue only happens if the game is configured to start outside of the dungeon, in which case normally a new character begins outdoors near the Privateer's Hold.
However if a character dies and waits through the death movie, and then a new character is made without restarting the executable the new character will start near where the previous character died instead (possibly the center of that square of the map?). It also seems at least some world state is retained, as the creature that killed the last character will still be present at the exact spot in the world they were in the last game and can be reencountered if the player returns to where they last died.

There are a number of things not being cleared by new games after death. For now, make sure to always restart the program before starting a new game.
