Quest length

No, those are rough groupings of hundreds of different factions in the game. They just provide a few general boosts to those groups, so it isn't possible to return to this same view and see how it's evolved.

I wonder how much work it would be then to have a dialogue option you could select when talking to someone in a town to get a rating of where the towns population rates you, similar to how you can see the reputation you have in guilds you are affiliated with. An entry in the tell me about menu might fit the bill, doesn't have to be a numerical value (although numbers would of course have to be occurring in the background) some like you are well thought of in Deerwood or it would be best if your shade never touched Deerwood soil.

Quest completion times are based on travel times; I guess this was a quest to a remote dungeon, and/or one of those quests where you have to visit several dungeons in sequence. Also if you're on foot, travel times are doubled, etc.

Internally they're actually over a hundred factions, see this thread on this topic.

Nope it was just a standard go to this dungeon in the same region that is 3 days away, kill the giant (that will suddenly become 6 or 7) then return. I wasn't on foot as I had the cart I think it was just an odd anomaly that occurred,it doesn't really hurt anything as you just end up with a quest in your log for a long time. Can always travel around to clear it or go to a different guild member who gives quests.
