It has nothing to with DREAM, I can say that much. Ive had it happen too when I was testing with only a terrain sampler mod. Havent been able to find the cause and logs dont show an error but I think it occurs when RMB layout fails to finish in time. Or maybe DFU thinks the location is fully loaded in cache already

I agree with Hazelnut's point: I don't recall this ever occurring without mods. I know we previously pointed to DREAM - Sound and its method of loading into the game; we'll have to find a common denominator. haloterm, if you can show the mod loadout you used when this occurred, that'd be a big help.
As I wrote in my first post, it did happen when I had Distant Terrain enabled, but the drawing distance was set to just 2. When I increased the drawing distance to 3, it did not happen again.

My manual travel was from Ripwych Commons to Copperfield Manor (in my BHC quest package there is a custom book which describes this hike in detail). The distance of this travel is 3 pixels, so I assume that increasing the draw distance to 3 fixed it because enough landscape was loaded. Basically what pango linked above.

I will do a systematic test later, incl. repeating the hike in pure vanilla, one time with a draw distance of 2, one time with a draw distance of 3. If it happens in any of these cases, it is not mod related, but draw distance related. If it does not happen then, it must be mod related. If so, I will repeat the test one time with Distant Terrain, one time with Mountains and Hills.

It can't be Distant Terrain specifically causing this issue, I use Mountains & Hills and it happened to me once. Having said that, who knows how many locations haven't spawned in properly but just go unnoticed in the background...

for reference: I think I found the cause of the issue (after pango found a way to reproduce it) - see this post: viewtopic.php?f=27&t=10&p=44347#p44347
it seems to be a problem of dfunity's terrain management strategy, not directly linked to terrain mods.
