Over time, all NPCs in all factions have been getting more and more hostile. It reached a point where I couldn't talk to anyone, so I tried using modnpcrep in the console to fix it. I got a message saying rep couldn't be increased due to an unknown error. I tried checking rep values in my save folder, but I'm not even seeing any negative values.
What's going on? I don't have any negative legal reputations, either.

Looking at your FactionData in the save file, you don't have negative rep with a lot of people. The worst offender is Dancers at -23 (not sure how that came to be), but you've apparently offended Betony (-8) and Daggerfall (-16), along with some of Daggerfall's nobles (-8).
The error with modnpcrep also continues on my end, and the player log doesn't illuminate the problem:

The "modnpcrep" console command is to be used on individual NPCs only (e.g. Barenziah) after clicking on the NPC you want to mod rep for. It's intended use is to bump rep to trigger main quests with a reputation minimum. It has no other purpose.
Your reputation is likely low for some valid reason (e.g. failed quests). It's hard to say without knowing play history. Legal status and faction reputation are separate things. You can be a common citizen in the eyes of the law and still generally disliked.

I've mostly stuck in Daggerfall completing Mage Guild quests with a few Thieves' Guild quests.
I don't think I've failed any quests, though, The closest I've gotten to failing a quest is turning in a person I was escorting to the town guards. Does it hurt to ignore certain quests?

It's hard to say. I think you have a record of your past quests completed/failed in your journal. If you take a look at that, theoretically there might be a way to retrace the change.
It's not always simple, though. Many of Daggerfall's factions are interlinking.
Furthermore, there are curious corner cases; if you ask the Fighters Guild questgiver for a commoner quest, the reputation changes that happen will happen to the Fighters Guild and its allies, not the commoners. So if "Dancers" is related to some faction you're working against consistently that might be it... But it's very hard to prove that. Other factions would likely reduce in sync as well.

Gotcha, I'll probably go back to an older save and do tavern quests. That might fix it.
