awesome mod, i wonder if this could increase the usefullness of the spymaster from thieves/DB. They always give accurate info as opposed to townsfolk being wrong a lot of the time.

This mod mainly adds to your sense of progression throughout the game, giving you a better idea of which regions you have explored more thoroughly.
You can no longer just pick and choose any location to go to, you have to be proactive to actually fill out the map, rather than all locations just being revealed for you. Travel Options and Basic Roads help with this as you can follow roads to give you a sense of where locations are to be discovered.
With this system I don't mind using fast travel, as you have to actually discover the location before being able to fast travel there. By default all cities are revealed on the map, but you can change this in the mod settings if you like.
Using this mod alongside the 'Prefer Nearby Quests' option in the Quest Offer Locations mod, keeps the location discovery more realistic as quests will no longer send you to the other side of the region. And then you can better see which parts of each region have been more throughly explored.

is discovery only connected to quests and physical discovery?
Could i ask townsfolk where the nearest X is and they would reveal the town or w/e on my map?

This is neat! I'll have to try this out.
This also gave me an idea for buyable / findable region maps that would reveal the (larger) towns in the appropriate region.

Yeah that kind of idea is in the 'Future Enhancements' section at the bottom of the mod description on Nexus,
I'll look at doing something like that when I get round to the next release
